Assessing Client Needs

Tri-County Hospital Home Health Care admission nurses make an initial assessment within 48 to 72 hours after receiving a referral from a physician and the client is home from a medical or care facility. Tender Home Care admission nurses coordinate with the patient and their family to make the initial assessment.

When meeting the admission criteria, Tri-County Hospital Home Health Nurses work closely with physicians evaluating and helping determine the best and most appropriate services to help clients get better. Reassessment occurs according to the physician’s plan of care and as the client’s condition warrants.

Home Health Care nurses believe nursing consists of assessing and meeting the client’s needs by:

  • Implementing and coordinating a plan of care designed by a registered nurse after consultation with client and their family and in compliance with the physician’s orders.

  • Assisting clients to establish and attain healthcare goals as described.

  • Educating clients in order to promote and maintain their health and goals, as well as taking responsibility for continuing those measures to attain, and maintain, the highest level of function.

  • Promoting client quality of life in all areas of care.
